HUNTER NEW ENGLAND, NSW — Uniting will introduce its renowned Continuing to be Me (C2bMe) program to Residential Aged Care Homes (RACHs) within the Hunter New England Central Coast Primary Health Network (HNECC PHN) area, enhancing mental health support for elderly residents.
The C2bMe program offers a personalized, evidence-based approach to care, focusing on the emotional well-being and clinical outcomes of aged care residents. Tailored to meet the unique needs of RACH residents, the program aims to ensure that each individual receives compassionate, dignified care.
The therapies provided through C2bMe are designed to:
- Reinforce personal identity and purpose
- Enhance social connection and instill hope
- Improve daily living
- Strengthen mental health and overall well-being
HNECC PHN has chosen Uniting to deliver the C2bMe program following a thorough and transparent tender process. With extensive experience in mental health care for older people, Uniting has previously implemented the program in RACHs across the South Western Sydney PHN and South Eastern NSW (Coordinare) regions.

Referrals for Uniting’s C2bMe program will open on April 1, 2025.
